The Pearl Rosenstein Center for
Rehabilitation- the bridge from hospital to home
The NEW Pearl Rosenstein Center for Rehabilitation at the Jewish Home for the Aged is designed to assist all individuals in attaining their maximum functional capacity. Caring for older adults with rehabilitative needs is a gentle art. It requires not only the science but also the skills of human compassion that nurture the healing process. Our goal is to integrate the patient back into the community with the ability to function at his/her highest level of independence. Nursing Services The nursing staff work hand in hand with our physicians and clinical team to encourage self care and independence for all residents based on their cognitive and physical ability. All residents are assessed to maximize their full potential and achieve individual goals. On admission, our interdisciplinary team create individualized care plans which address each person's physical, spiritual, emotional, intellectual and recreational needs. Ongoing discussion with residents and family members creates an atmosphere of positive communication, shared goals and care. ![]() Medicine We continue to strive to be the pinnacle of health care available to the elderly we serve. The Jewish Home has pioneered a new approach to medical care delivery. A team of Geriatricians, physicians and nurse practitioners are full time employees of the Home and treat each resident. This ensures continuity, commitment and competence. Now, our response to an acute medical episode or change in condition can be footsteps away. Interdisciplinary Team Approach Residents of the Jewish Home are individuals comprised of a full mosaic of lifetime experiences. Each of our clinical departments are staffed with liscenced professionals who bring their expertise to developing a comprehensive plan of care addressing each aspect of wellness. The team is comprised of the Physician, Nurse, Social Worker, Recreation Therapist, Dietician and Rehabilitation Therapists.
